Adventist Health Home Care Services

Adventist Health Home Care Services Hospice works together to create a strong bond between the patient, their loved ones, their physician, and our team. Care is focused on the whole person, mind, body, and spirit. Services include physician and pharmacy consultation, medications for symptom management, medical equipment and supplies, personal care assistance, emotional support and assistance, bereavement counseling and grief support for family members and caregivers, 24-hour on-call medical staff and nurse support, spiritual support, and end of life planning. Our team is united by a passion to make a difference in the lives of those we serve by providing personalized care and support. Our expert team includes our Hospice Medical Director, registered nurses, social workers, home health aides, chaplains, grief counseling, and trained volunteers.

Volunteers:

Hospice volunteers provide companionship for patients in their homes or facilities, as well as respite relief for family and caregivers. Through compassionate presence and availability, volunteers are an integral part of the staff, to ensure patients and families are provided the care and support they need during the final days of their loved one's lives.

Bereavement volunteers provide assistance with processing letters that are mailed to the bereaved and help organize and assemble patient admission packets and various brochures. Bereavement volunteers also assist with grief group facilitation.
